WTB Burris extension rings

This is a long shot but I need a set of these. Must be the ones as pictured. The only place I could find a picture of them was in Midway and the link above is the picture. Unfortunately they are discontinued and no longer made.
I am able to get the ones with the dovetail extension from several sources but I need the ones with the extension on the windage ring.

Can be blue or silver but must be medium height.
